Job Title: Home Care Registered Nurse
Department: Home Care
Reports To: RN Supervisor
Status: Non-exempt, PRN status as-needed

SUMMARY: The home care nurse is responsible for the delivery of quality patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. The nurse is responsible for oversees and directs patient care, case management, documentation and customer satisfaction of all patient care services. Duties include assessing the patient's home environment, provide care and instruct the patient and families. The home care patient can include those recovering from illness, accidents, surgery as well as the disabled, chronically ill, or cognitively impaired.
ESSENTIAL KNOWLEDGE, SKILLS AND RESPONSIBILITIES include the following. Other duties may be assigned.

1. Ability to work independently and collaboratively to provide and direct skilled comprehensive nursing care and instruction for patients and families based on established clinical nursing practice standards.

This includes but not limited to:
? Administering care, treatments, m edication in compliance with physician's orders
? Monitoring and interpreting symptoms diagnostic test and treatments developing a plan of care in collaboration with other health care providers
? Providing education to patients and families
2. Responsible for assessing patient referrals and completing admissions and home visits as assigned. This includes:
? Initiating the plan of care and priority setting to ensure effective and efficient patient care deliver and achievement.
? Consulting and coordinating with the health care team members to assess, plan, implement and evaluate patient's plan of care.
? Following a plan of care that assures the patient and family needs are identified, prioritized, and met in order to achieve desired clinical outcomes/goals.
? Regularly reevaluates the patient's nursing needs and makes the necessary revisions in the plan of care.
3. Assures that care, educational and discharge planning needs of patients are met .
?Teaches patients and families preventative care, disease prevention, medical and psychological care, and medication review.
?Monitors ongoing patient care, through care conferencing and review of documentation, to determine care, educational and discharge planning needs.
?Utilizes knowledge of patient's age and cultural diversity in the provision of patient care and education needs.
4. Evaluates discharge outcomes to look for improvements or decline and any discrepancies.
5. Directs and supervises, including supervisory visits, within the area of responsibility including Licensed Practical Nurses (LPN) and Certified Home Health Aides (CHHA).
6. Communicates a report to the appropriate health care team members for the continuation of care, including reporting any changes in patient's condition to the physician and family as needed.
7. Demonstrates the knowledge and skills necessary to provide care appropriate to patients of all age groups?must d emonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient's status, and interpret the appropriate information needed to identify each patient's needs.
8. Maintains universal precautions guidelines and safety measures according to RiverStone Health policies and procedures
9. Documents care provided for the patient encounter in the electronic medical records (per documentation standards) in a timely manner. Must document/chart observations, activities and other data related to the clinical condition of the patient and patient care.
10. Experienced in Outcome and Assessment Information Set, OASIS-C, quality and reimbursement measures which include structure, process and outcomes.
11. Contributes to the provision of quality process through performance improvement techniques that demonstrates positive outcomes in patient care. Receives and works to resolve patient/family con cerns/complaints.
12. Maintain knowledge of and compliance with current Medicare/Medicaid, state regulations and rules, Health Insurance Portability and Accountability Act (HIPAA) compliance and accreditation regulations for home health services.
13. Maintain and enhance professional growth and development through attendance in orientation, continuing educational offerings as well as organizational educational programs as requested.
14. Demonstrates interpersonal, cognitive, and technical competencies and standards of care consistent with the program's and organization's mission, values and services.
15. Ability to help cover needs in home care, private duty and hospice as assigned.
16. Must carry a program computer and telephone to receive and place calls for patient care operations during non-office hours when on call. QUALIFICATIONS to perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requireme nts listed below are representative of the knowledge, skill, and/or ability required. EDUCATION AND EXPERIENCE
1. Graduate from an accredited school with a nursing degree.
2. Minimum two years nursing experience.
3. Proficient oral and written communication skills aimed at a variety of patients, families and health care team members.
4. Exceptional knowledge of nursing process and home care regulations.
5. Required proficiency with personal computer and keyboarding essential and be willing to learn Hospice documentation software applications including the electron medical record.

CERTIFICATES, LICENSES, REGISTRATIONS:
1. Current Montana State licensure as a Registered Nurse
2. Current CPR certification
3. Valid Montana Driver's License in good standing with an automobile that is insured in accordance with t he State of Montana.
