In this position, the Valve Clinic Coordinator (VCC) conducts the retrospective echocardiogram review to identify specific opportunities for improvement! .
This serves as the basis for the VCC to draft guideline-based procedures and protocols to promote a more systematic and objective approach to patient identification and evaluation for valve disease. The VCC helps to drive a quality based, patient-focused program which is devoted to appropriate management of the population with valvular heart disease.
Following the implementation of key protocols and processes the VCC is responsible for managing referrals, communicating patient information to all providers, scheduling appropriate evaluations and follow-ups, as well as, managing the data base to track all relevant valve clinic patient information and activity.
Essential Duties and Responsibilities
Program Development and Support

In conjunction with the Director of Cardiovascular Service Line, is responsible for program coordination and development of Echo Alert system within the walls of
St. Vincent Healthcar! e
·
Coordinates with Primary Care Provider,! Cardiology and Cardiothoracic surgery to facilitate appropriate patient follow up and determination of care. Based on the ACC/AHA guidelines for follow-up echocardiography or advancement of symptoms

Provides participating physician and clinical office staff with ongoing support as needed regarding individual program guidelines of the valve clinic

Educate referral network on the valve clinic function and benefit to their patients and practice
·
Interfaces
appropriately and
professionally with employed and
independent clinics, other hospital departments, outlying referral hospitals, clinics and physicians

Assists in promoting community awareness of the
program/services
·
Promote patient disease awareness
·
Participates in community outreach activities and provides marketing assistance for program activities and special ev! ents
Research and Data Tracking
Conducts the retrospective echocardiogram reviews and addresses review findings
Supports research activities involving the Cardiac Valve population.
Creates patient data base to ensure appropriate tracking of patients.
Manages the valve data collection/registry and provides graphs and trend reporting on a quarterly basis
Maintain statistical and database entry and tracking for structural heart program
Assists the Cardiovascular Service Line Director with tracking and development of
revenue return on investment opportunities in conjunction with marketing guidelines
Valve Program Coordination
Collaborates with the Cardiovascular Service Line Providers to coordinate valve clinic structure
Collects all potential patient data from these various channels; Echo alert, direct from PCP, direct from cardiology, community outreach
Communicates wit! h PCP or other attending physicians willingness to assist with referral! coordination for patients who meet established criteria based on ACC/AHA guidelines for follow-up echocardiography
Coordinates valve clinic consults with multidisciplinary team for evaluation
Communicates back to referring physician highlighting the issues presented; discussion between cardiology and surgery and treatment consensus
Clinical Care Coordination
Assesses patient and family satisfaction related to care

Coordinates inpatient care and acts as a liaison with Primary Care physicians, Cardiothoracic surgeons,
Cardiologists, nursing and therapies

Oversees the patientâs progression through the hospital system and assists with answering patient/family questions
Education, Experience and Licensure/Certification Requirements:
Requirements are
representative of the minimum level of knowledge, skills and/or abilities necessary to perform the essential functio! ns of the position
1.
Current MT RN license or RDCS/RCS registered.
2.
Current BLS certification
3.
Recent work experience in a cardiac related department (ICU, Telemetry, Cath Lab, CV Clinic, etc.).
4.
Ability to create and manage database information, create and interpret graphs and reports.
Ability to communicate effectively and supportively within a multi-discpilinary team
Strong organizational skills and attention to detail

Experience and
understand and usage of computers and the ability to learn applications relevant to the position
