The Women's Health Clinical Navigator (WHCN) is a registered nurse responsible for identifying, facilitating, and coordinating pat! ient-centered care options and services between VA and authorized Non-VA Care/Fee Base providers and patients. The WHCN has a crucial role in assisting women Veterans to efficiently obtain gender specific services and care, with emphasis on maternity, gynecology, and breast care coordination and follow-up. The WHCN functions as a liaison and is responsible for planning, coordinating, and performing a wide range of case management, clinical, administrative, program, and technical functions to support quality gender-specific services for women Veterans. The WHCN works directly with patients, VA Staff, Non-VA care providers and organizations, Radiology Department, and the Women Veteran's Program Manager to achieve seamless care, to facilitate patient satisfaction to reduce duplication of services or delay of care, and to monitor quality of care and services received. The registered nurse is responsible and accountable for the provision of direct and indirect patient care and f! or the delivery of quality nursing care to culturally-diverse,! female, Veteran patient population. The registered nurse possesses the knowledge and skills to effectively apply all aspects of the nursing process with collaborative, interdisciplinary practice, which will then enable effective care coordination and implementation of health promotion and disease prevention practices. Function as the Women's Health contact for staff and patients when the Women Veteran Program Manager is not available. Performing other duties as assigned.
Summary
Fort Harrison (Helena) is a 48-bed acute care, medical-surgical facility that offers a broad range of acute, chronic, and specialized inpatient and outpatient services for both male and female veterans. Specialty care includes internal medicine, gerontology, neurology, dermatology, cardiology, palliative care, pain management, medical oncology, surgery (general, vascular, laparoscopic, endoscopic), urology, orthopedics, plastic, ophthalmology, ENT, podiatry, gynecology, chiropract! ic care, psychiatry (including outpatient substance abuse treatment and PTSD and MST specific care), and ambulatory care (primary care). Radiology Service provides a broad range of diagnostic and interventional care provided on a full time basis. Pathology Services are available on site also provided on a full time basis. Telemedicine services are available for psychiatry, radiology, gynecology, primary care, ophthalmology and tele-home health. The 24-bed inpatient mental health facility serves patients needing treatment for PTSD, Substance Abuse and Acute Care.
